Financial and Appointment Policy
Pricing, payment, insurance, cancellations and what we ask of you as a patient. Effective October 6, 2026.
Pricing and payment
- Our prices are published on this website and confirmed at your consultation. You are told the exact cost of your plan before anything is scheduled or opened, and the price does not change afterward.
- Under federal law, patients who don't have certain types of health care coverage, or who are not using that coverage, have the right to receive a Good Faith Estimate of the expected cost of their care on request or when scheduling, as set out in our Good Faith Estimate notice.
- The new patient consultation is $250. Cosmetic treatments are priced per unit, per syringe, per vial or per session, so you pay only for what is used. We do not sell packages or memberships.
- Payment is due in full at the time of service. We accept cash, checks, major credit and debit cards, and HSA and FSA cards for medical and surgical care. Financing is available through Cherry; see our insurance and financing page.
- Cosmetic services and products are non-refundable once treatment has been provided or a product has been opened for you. Results vary, and a result that does not meet expectations is addressed at a follow-up visit, not by refund.
Insurance
- ZN Dermatology is out of network with all commercial insurance plans. Most of the care we provide is cosmetic and is not covered by any insurer.
- For medical and surgical visits, we provide an itemized superbill on request so you can seek reimbursement from your plan under your out-of-network benefits. Reimbursement is your plan's decision and cannot be guaranteed.
- Original Medicare. Accepted. We file the claim for you. You pay our Medicare rate at your visit, an amount Medicare sets, and Medicare sends its share directly to you.
- Medicare Advantage PPO. We are out of network, but most PPO plans cover out-of-network care. When your plan covers the visit, we file the claim and you pay only your plan's share at the visit: your deductible, copay or coinsurance.
- Medicare Advantage HMO. We are out of network, and most HMO plans do not cover non-emergency care outside their network unless they approve it first. If your plan covers the visit, you pay only your plan's share; if not, you pay our published price. Call the number on your card before you book.
- Pathology laboratories bill separately for reading biopsies and usually bill your insurance directly. Bring your insurance card to every medical visit.
Appointments
- Please give us at least 48 hours' notice to cancel or reschedule so the time can be offered to another patient. We do not charge cancellation fees.
- Repeated no-shows or late cancellations may result in scheduling restrictions, such as a request to confirm by phone before future visits, and in rare cases in dismissal from the practice.
- Please arrive on time. Because visits are spaced so that patients rarely overlap, a late arrival may need to be shortened or rescheduled to respect the next patient.
- Consent forms are completed electronically before your visit through the patient portal. Financial and general consent are renewed yearly; procedure consents are signed for each procedure as our consent schedule requires.
Prescriptions
We do not submit prior authorizations for routine prescriptions. Prescriptions are sent to the pharmacy of your choice with a GoodRx coupon code so they can be filled the same day, usually for about $35. Where a medication is costly enough that this route does not make sense, we discuss alternatives or, on request, submit a prior authorization to your insurer. See Prescriptions for details.
Medical records
You may request copies of your records at any time; see the Notice of Privacy Practices for your rights and any applicable fees.
Respect
We treat every patient with courtesy and expect the same for our staff and other patients. Abusive or threatening behavior toward staff may result in dismissal from the practice.
